About Us
Oak Supportive is a specialised Mental Health and Social Care provider with many years of valuable experience. Our main aim is to deliver much needed health and social care to clients based in and around North Liverpool.
Having accrued many years of experience gaining the great wealth and knowledge needed to support our valued and at times vulnerable and challenging clients, it’s equally important that they can trust us to support and look after them, so all of our team have full up today DBS (disclosure and barring service) checks and documentation.
Our Staff
All our staff work as a team to deliver person centred support based around the individual needs of the client, identifying what’s needed to enable them to live as independently as possible. All our staff are fully trained and have a Health & Social Care diploma or are working towards their qualification to ensure that our clients can live a safe and happy life in the comfort of their own homes.
Areas of support we can assist with
Each of our clients will have a tailored support plan outlining their support needs, it is developed with the individual in mind to ensure that their needs are met to the highest of standard.
- Support going shopping (encouraged to make good food choices and how to budget)
- Activity support such as a trip to the cinema, bowling, library, hairdresser/barbers etc….
- Support learning everyday life skills, such as domestic assistance
- We can assist in arranging and attending dentist/doctor/hospital appointments etc…
- Assist with benefit advice and support in completing forms
- All aspects of them maintaining a tenancy agreement
- Sourcing & accompany clients to community & recreational activities to enable them to be able to take part in any interests and hobbies they may have.
- Encourage & assist with visiting family and friends
- Support and advise with clients who have pets
- Or simply just being there to have chat, have lunch or a cup of tea
